3. Certificate of Registration or Certification of enrolment for Enrolled Grade 12 Students
4. Certificate of Good Moral Character
5. Photocopy of PSA Birth Certificate
6. Certificate of Indigency (if applicable -Annual Income Tax Return of Parents (if applicable)
7. 1pc. 2x2 sure latest ID photo wo nametag & white background

FOR TRANSFEREES

1. Duly Accomplished application Form
2. Transcript of Records or Certification of Grades
3. Certificate of Good Moral Character
4. Photocopy of PSA Birth Certificate

FOR SECOND COURSER | SECOND DEGREE APPLICANT

FOR FOREIGN STUDENTS

3. Student's Personal History Statement (Duly signed by the applicant, both English and his her national alphabet accompanied by his her personal seal if any, containing among others, his her left and right thumb-prints)
4. Transcript of Records Scholastic Records and Diploma or Certificate of Completion with English Translation)
5. Notarized Affidavit of support;
6. Birth Certificate or its equivalent;
7. Medical Health Certificate Authenticated Police Clearance Report
8. Authenticated police clearance / report from the country of origin:
